Prognostic Factors of Colo-rectal Cancer



Abstract
This report is a retrospective review of colorectal cancer in 139 follow-up cases among 160 cases of curatively resected patients who were admitted to the Department of Surgery, Seoul National University Hospital from January 1973 to December 1978.
We mainly studied about the 5-year survival rates and the factors affecting the prognosis for the purpose of evaluating the practical situations of carcinoma of colon and rectum in Korea and its clinical behaviors. The results are as follows.
1) The peak incidence occurred in 5th decade of life, 24.4% of all patients were below 39 years of age, and male to female ratio was 1.4: 1.
2) The 5-year survival rate according to the age was 38.2% for the patients under age of
39 years as compared with 61.0% for the patients over 40 years.
3) Male had a 54.8% of 5-year survival rate, and female had a 56.1% of 5-year survival rate.
4) As to survival rate according to the location of primary lesion, 5-year survival rate was 48.9% for the patients with lesion in rectum, 65.4% in left colon, and 68.0% in right colon.
5) As to survival rate according to the size of primary lesion, 5 year survival rate was 71.4% for the patients with 0-2.0 cm in diameter, 42.9% with 2.1~3.0 cm in diameter, 53.8% with 3.1-5.0 cm in diameter, and 62.3% with over 5.1 cm in diameter.
6) As to survival rate according to the gross configuration, 5-year survival rate was 77. 1% for the patients with fungating type, 48.9% with ulcerating type, and 47.4% with intermediate type.
7) As to survival rate according to the degree of obstruction, 5-year survival rate was 60.2% for the patients with mild degree of obstruction, 39.2% with moderate degree, and 69.2% with severe degree.
8) As to survival rate according to the histologic type, 5-year survival rate was 59.9% for the patients with adenocarcinoma, 38.9% with mucinous adenocarcinoma, 25.5% with squamous cell carcinoma, and 0% with leiomyosarcoma. As to survival rate according to the degree of differentiation among the 116 patients with adenocarcinoma, 5-year survival rate was 64.8% for the patients with well differentia-tion, 50. 0% with moderate differentiation, and 50.0% with poorly differentiation.
9) As to survival rate of 88 rectal cancer patients according to the type of operation, patient who received polypectomy or low anterior resection was one respectively and all had survived over 5 years after operation. Patients who received Miles operation had a 40.6% chance of 5 year survival as compaired with 68.2% for the patients who received pull thr-ough operation.
10) As to survival rate according to the extent of the lesions, 5 year survial rate was100% for the patients with Astler-Coller modified Dukes¢¥ A lesion, 80.8% for Bl lesion, 65.2% for B2 lesion, 62.5% for Cl lesion, and 33.3% for C2 lesion.
